What is a PCN?
A Primary Care Network is a group of GP practices who remain independent organisations but share some funding and some staff in order to work more closely together.
These networks enable General Practice to work more closely with community, mental health, social care, pharmacy, hospital and voluntary services in their local areas. In terms of size, in Coastal West Sussex, these networks range from 1-9 practices and can cover between 28,000 – 100,000 patients
